Care Plans may be self-funded by families or provided at no cost through school, clinic, or organizational partnerships.

Southern Queer Collective offers short-term, non-clinical Care Plans for queer youth, young adults, and their families during moments of emotional distress, school conflict, identity-related stress, or system overwhelm.

Care Plans are designed to provide clarity, structure, and practical guidance—without ongoing sessions, open-ended commitments, or therapy.

Each Care Plan combines:

  • care navigation across relevant systems

  • customized written support plans you can use immediately

How Care Plans Work

Care Plans are:

  • clearly scoped before work begins

  • short-term and finite

  • delivered primarily through planning, research, and written guidance

  • designed to stand on their own once completed

This approach allows for meaningful support without creating long-term dependency.

Care Plan Options & Fees

The Care Plan type is determined collaboratively after intake, based on the situation and level of complexity.

Single-Issue Care Plan

$950

Best for one clearly defined concern.

Includes:

  • care navigation focused on a single issue

  • 1 customized deliverable, such as:

    • crisis-prevention guidance

    • a school advocacy packet

    • a family communication roadmap

  • limited clarification once the plan is delivered

Common uses:

  • a specific school concern

  • early signs of emotional escalation

  • guidance for a single transition

Comprehensive Care Plan (Most Common)

Best when multiple systems or concerns overlap.

Includes:

  • broader care navigation across systems

  • 2–3 customized deliverables, such as:

    • crisis-prevention and safety guidance

    • school advocacy and accommodation planning

    • identity-affirming support guidance

    • family communication and boundary planning

Common uses:

  • school + emotional distress

  • identity-related stress affecting multiple environments

  • caregiver overwhelm and uncertainty about next steps

Complex Systems Care Plan

Best for high-stakes or multi-layer situations requiring deeper planning.

Includes:

  • extended care navigation across multiple systems

  • 3–4 customized deliverables

  • more extensive research, mapping, and coordination guidance

Common uses:

  • repeated escalation or near-crisis situations

  • school refusal or reintegration planning

  • complex intersections of school, family, and mental health systems

Southern Queer Collective does not provide:

  • therapy or clinical mental health treatment

  • crisis or emergency response

  • ongoing counseling or weekly sessions

  • on-call availability

This structure allows care to remain ethical, effective, and sustainable.
